New Clients

 At House of Dog Countryside Retreat we take necessary precautions to ensure the health and safety of all dogs in our care.  

All dogs entering the House of Dog for services and/or accommodations must provide current documentation on the following health requirements: 

 Requirements:

- Distempter/Parvo or proof of titers

- Rabies vaccination certificate

- Annual Bordetella vaccine

- Flea and tick preventative medication

To start the registration process, all new clients must fill out an online application and sign our Agreement and Medical Release. You will also be required to upload a copy of the following: 

 -Rabie's certificate- signed by a PA licensed vet (out of state certificates will not be accepted). 

- A copy of your dog's current vaccination record. (Bordatella & DAPP required)

Please have all documents available to upload at time of registration or the submission will not be processed.

***If your dog is over the age of 10, we also require you to fill out our rainbow policy.
